►  Pick up a “Physical Packet” from the Athletics Office or Athletic Training Room in the Field House.

►  Have the Medical History portion of the Physical Form filled out/signed by your parent/guardian BEFORE you come for the physical.

If you have Asthma or Severe Food/Latex Allergies, additional forms will be needed/please see one of the school nurses.
