On Saturday, May 14th, 2022 at the Ballad Sullivan Center - Outpatient Rehabilitation & Physical Therapy on Stone Drive. Ballad Health will be offering free athletic physicals for all prospective D-B athletes, including rising 9th graders for the 2022-2023 school year.


Ballad Health is requiring Pre-Registration for the physicals. Complete this sign up form to register for the physicals (CLICK HERE). Please complete this form by May 6th and bring your completed KCS Medical Authorization packet with you on the morning of the physicals. Prior to the morning of the physicals, you will receive your arrival/appointment time via email. Please arrive at your arrival/appointment time on time with your KCS Medical Authorization packet signed and completed.


KCS Medical Authorization packets are attached to this email but may also be printed out from the KCS Athletics website under athletic forms or picked up from the Athletic Office at Dobyns-Bennett High School.


Physicals will be conducted on May 14, 2022 from 7:45AM - 9:30AM at the following address:
